In-network. Some HMSA plans require a referral from your primary care provider; we check yours before the first visit.

In-network with QUEST (Hawaii Medicaid). We confirm coverage before your first visit.

How a virtual visit works
Most of pelvic floor physical therapy is education, breathing and pressure management, and guided movement. That is why it translates so well to video.

Discovery call
15 minutes, free, by phoneA short call to hear what is going on, explain how visits work, and check insurance or pricing. No pressure to book.
Evaluation
60 minutes, by videoDr. Shalem takes a full history, watches how you move and breathe, and guides a self-assessment so she can see how the pelvic floor is working.
Your plan
Same visitYou leave the evaluation with a plain-language explanation and a short home program, usually a few minutes a day.
Follow-ups
45 minutes, every 1 to 2 weeksProgress checks, program updates, and techniques you can do yourself, for as long as they are useful.

Common questions in Hawaii
I'm on TRICARE Select. What do I need to book?
Nothing beyond your TRICARE information. Select members can see a network provider without a referral. Book the discovery call, and we will verify eligibility before your evaluation.
I'm on TRICARE Prime. Can I still see you?
Yes, with a referral from your primary care manager. Ask your PCM for a pelvic floor physical therapy referral, or book a discovery call and we will walk you through the exact steps.
Do I have to be on Oahu?
No. Video visits are available anywhere in Hawaii, including the neighbor islands.
What if we PCS before I'm finished?
Care can continue by video in any of the seven states where Dr. Shalem is licensed: Florida, Hawaii, Maryland, New York, California, Oregon, and Montana. Coverage may change with the move, and we will help you sort it out.
What does a visit cost with insurance?
That depends on your plan's copay or cost share for physical therapy. We check your benefits before the first visit so there are no surprises.
Does pelvic floor physical therapy work over video?
Much of pelvic floor PT is education, breathing and pressure management, movement retraining, and a home program, and all of it works on camera. During the evaluation Dr. Shalem guides a self-assessment so she can see how the pelvic floor is coordinating.
An internal examination is not possible by video. If a hands-on assessment would change the plan, she will say so and help you find local care.
