Physical therapy after hysterectomy and pelvic surgery in Maui, Molokai and Lanai

Hysterectomy, prolapse repair, bladder sling, and endometriosis surgery all change how the pelvic floor and core work. A structured rehab plan protects the repair and gets people back to lifting, exercise, and intimacy with confidence.

About the condition

Recovery after pelvic surgery: what is going on.

Pelvic surgery is common, and rehab after it is rare. Many women leave the hospital with a list of things not to do for six weeks and no plan for what comes after. Scar tissue, a guarded pelvic floor, a core that has switched off, and fear of harming the repair are the usual results. For prolapse and incontinence surgery in particular, how the pelvic floor is used afterward affects how long the repair lasts.

Post-surgical pelvic floor physical therapy restores coordination and strength in stages, teaches pressure management that protects the repair, and addresses scar tissue and pain.

Common experiences

What people with recovery after pelvic surgery often describe

These are the patterns Dr. Shalem hears most often. Any of them is a reason to book a discovery call.

Weakness or disconnection in the core after surgery

Pain or tightness around incisions

Fear of lifting, exercising, or intimacy after a repair

Leaking or urgency that persists or appears after surgery

Constipation and straining after surgery

Uncertainty about what is safe and when

Treatment

How physical therapy addresses it.

The program starts with breathing, gentle pelvic floor and core activation, and bowel and bladder habits that avoid straining, then progresses to scar mobility, strengthening, and a staged return to lifting and exercise within the surgeon's restrictions. Dr. Shalem coordinates with the surgeon's protocol.

What a video visit looks like

Questions

Recovery after pelvic surgery questions in Maui, Molokai and Lanai

When can rehab start after pelvic surgery?

Breathing, gentle activation, and bowel habits can start within the first weeks with the surgeon's clearance. Strengthening progresses after the six-week check.

Can physical therapy help a prolapse repair last longer?

Learning to manage pressure during lifting, coughing, and exercise reduces the load on the repair, and pelvic floor strength supports it. That is why many surgeons recommend physical therapy after prolapse surgery.

I had surgery years ago and still have symptoms. Is it too late?

No. Scar tissue, pelvic floor coordination, and core strength all respond to treatment at any point.

Do I need a referral for physical therapy in Hawaii?

Hawaii allows you to start physical therapy without a physician referral. Your insurance plan may still ask for one (TRICARE Prime and some HMSA plans do), and we check that for you before the first visit.
