Insurance and referrals

Do I need a referral for pelvic floor physical therapy?

Usually not to start. Every US state allows some form of direct access to physical therapy without a physician referral, though some limit it to a number of days or visits. Insurance plans are the other factor: HMO plans, TRICARE Prime, and some Medicaid plans require a referral even where the state does not.

What to know

Answered by the practice of Dr. Elana Shalem, DPT, pelvic floor and orthopedic physical therapist.

State rules: Hawaii, Maryland, Oregon, and Montana allow evaluation and treatment without a referral and without a time limit. Florida allows 30 days, New York 10 visits or 30 days, and California 45 days or 12 visits before a physician's sign-off is needed to continue.

Plan rules: PPO plans and TRICARE Select generally do not require a referral. HMO plans, TRICARE Prime, and some Medicaid managed care plans do, and in those cases the referral has to come from the primary care provider before the first visit.

A practice that verifies benefits checks both before the evaluation and tells the patient exactly what to ask the doctor for. For most new patients, the answer is that they can book directly.

How it works

How a virtual visit works

Most of pelvic floor physical therapy is education, breathing and pressure management, movement retraining, and a home program. All of it works on camera. During the evaluation, Dr. Shalem guides a self-assessment so she can see how the pelvic floor is coordinating with breath and movement. An internal examination is not possible by video; if a hands-on assessment would change the plan, she says so and helps the patient find local care.
