Can diastasis recti be treated with physical therapy over video?

Yes. Dr. Shalem teaches a self-check of the midline on camera, watches how the abdomen responds to sitting up, lifting, and breathing, and adjusts the program visit by visit.

What to know

Answered by the practice of Dr. Elana Shalem, DPT, pelvic floor and orthopedic physical therapist.

During pregnancy the connective tissue down the middle of the abdomen stretches to make room. For many women it stays wider afterward, and the midline bulges or domes when they sit up or lift. Diastasis is not a tear and not a hernia, but it changes how the abdominal wall manages pressure, and it often travels with low back pain, pelvic floor symptoms, and a core that feels weak no matter how many crunches are done.

The program retrains breathing and pressure management first, then builds deep abdominal activation, then progresses to loaded movements, planks, and the patient's own sport. Crunches and sit-ups are reintroduced only once the midline can hold tension. Most of the work is a few minutes a day at home.

An internal examination is not possible by video, and a small number of patients need hands-on treatment or imaging first; the therapist says so at the evaluation and helps the patient find local care rather than continuing by video.

At EMS Therapy the evaluation is 60 minutes and follow-ups are 45, by video, one-on-one with Dr. Elana Shalem, DPT. A free 15-minute discovery call comes first.

How it works

How a virtual visit works

Most of pelvic floor physical therapy is education, breathing and pressure management, movement retraining, and a home program. All of it works on camera. During the evaluation, Dr. Shalem guides a self-assessment so she can see how the pelvic floor is coordinating with breath and movement. An internal examination is not possible by video; if a hands-on assessment would change the plan, she says so and helps the patient find local care.
