Men's pelvic health · Kauai

Pelvic floor physical therapy for urgency, frequency and leaking in men in Kauai

Rushing to the bathroom, waking at night, and dribbling after urinating are common in men and are usually blamed on the prostate. Often the pelvic floor and bladder habits are the bigger part, and both are treatable.

About the condition

Male urgency, frequency and leaking: what is going on.

Urgency and frequency in men can come from an enlarged prostate, an overactive bladder, a pelvic floor that never fully relaxes, or all three. Post-void dribble, a few drops after finishing, is usually a pelvic floor problem rather than a prostate one. Leaking with coughing or exertion is common after prostate procedures. Many men are offered medication or surgery without anyone looking at the muscles.

Behavioral treatment and pelvic floor training are first-line care for overactive bladder in urology guidelines, and they are used alongside prostate treatment when that is also needed.

Treatment

How physical therapy addresses it.

The program includes urge suppression techniques, a schedule that lengthens the time between voids, pelvic floor coordination for both control and relaxation, a technique that stops post-void dribble, and fluid and dietary adjustments. A bladder diary tracks progress. Dr. Shalem coordinates with the urologist when prostate treatment is also in play.

What a video visit looks like

Bladder retraining and pelvic floor coordination are taught and progressed entirely by video. Dr. Shalem reviews the diary each visit and adjusts the plan.

Questions

Male urgency, frequency and leaking questions in Kauai

Is this the prostate or the pelvic floor?

Often both. The urologist evaluates the prostate; physical therapy treats the pelvic floor and bladder habits. Many men improve with the muscle work alone, and the two treatments work together.

Can post-void dribble be fixed?

Usually, with a specific pelvic floor technique that empties the urethra after urinating. It is one of the quickest things to improve.

Will I need medication?

Behavioral treatment is tried first in guidelines. Whether medication is also needed is a decision for the patient and their physician.

Do I need a referral for physical therapy in Hawaii?

Hawaii allows you to start physical therapy without a physician referral. Your insurance plan may still ask for one (TRICARE Prime and some HMSA plans do), and we check that for you before the first visit.
