Video visits
What happens at a pelvic floor physical therapy evaluation?
A pelvic floor evaluation is a 60-minute visit that covers a full history, a look at posture, breathing, and movement, and an assessment of how the pelvic floor contracts and relaxes. By video, the assessment is self-guided with the therapist coaching; in a clinic it may include an internal exam.

What to know

The history takes the most time and matters the most. The therapist asks about bladder and bowel habits, pregnancies and births, surgeries, pain, intimacy, exercise, and what the patient wants to get back to. Most pelvic floor problems are explained largely by that conversation.
The movement assessment looks at how the patient breathes, stands, bends, squats, and lifts, and how the abdomen and pelvic floor respond to pressure, for example during a cough. Over video, the patient is coached through a self-assessment of pelvic floor contraction and relaxation, usually lying down and fully clothed, so the therapist can learn whether the muscles are weak, overactive, or poorly coordinated.
The visit ends with a plain-language explanation of what is going on and a first home program, typically a few minutes a day. The plan is adjusted at each follow-up.
At EMS Therapy, the evaluation is preceded by a free 15-minute phone call, so insurance, pricing, and whether video is a good fit are settled before the first visit.

How it works
How a virtual visit works
Most of pelvic floor physical therapy is education, breathing and pressure management, movement retraining, and a home program. All of it works on camera. During the evaluation, Dr. Shalem guides a self-assessment so she can see how the pelvic floor is coordinating with breath and movement. An internal examination is not possible by video; if a hands-on assessment would change the plan, she says so and helps the patient find local care.
