Orthopedic

Can physical therapy help with headaches that start in the neck?

Yes. Cervicogenic headaches begin in the upper neck joints and muscles and are felt at the back of the head, behind the eye, or at the temple. They respond to physical therapy that restores upper back and neck mobility, strengthens the deep neck muscles, and fixes the workstation and sleep habits that keep the pattern going.

What to know

Answered by the practice of Dr. Elana Shalem, DPT, pelvic floor and orthopedic physical therapist.

The neck carries the head over a thoracic spine that, for most adults, has stiffened from years of sitting. The result is a forward head, overworked muscles at the base of the skull, and headaches that build through the workday. Turning the head is stiff, and the upper shoulders ache by evening.

The program includes mobility work for the upper back and neck, deep neck flexor and shoulder blade strengthening, and specific workstation, pillow, and screen adjustments. Headache frequency and neck rotation are tracked as progress markers.

Over video, the therapist can see the actual desk setup, which is often the most useful part of the evaluation.

How it works

How a virtual visit works

Most of pelvic floor physical therapy is education, breathing and pressure management, movement retraining, and a home program. All of it works on camera. During the evaluation, Dr. Shalem guides a self-assessment so she can see how the pelvic floor is coordinating with breath and movement. An internal examination is not possible by video; if a hands-on assessment would change the plan, she says so and helps the patient find local care.
