Men's pelvic health

Can I start pelvic floor therapy before prostate surgery?

Yes, and it is the best time. Learning the correct pelvic floor contraction before a prostatectomy, while sensation is intact, leads to faster return of continence afterward. One or two pre-surgery visits are usually enough; training resumes as soon as the catheter is removed.

What to know

Answered by the practice of Dr. Elana Shalem, DPT, pelvic floor and orthopedic physical therapist.

Before surgery the therapist teaches the patient to find and isolate the pelvic floor, to contract without bearing down or clenching the buttocks, and to relax fully. The patient practices daily until surgery.

After the catheter comes out, with the surgeon's clearance, the program restarts at a low level and builds: endurance holds, quick contractions timed to standing and coughing, and walking progression. Bladder habits and fluid timing are adjusted.

Video visits mean the pre-surgery session can be scheduled within days, and post-surgery visits happen from home while the patient is recovering.

How it works

How a virtual visit works

Most of pelvic floor physical therapy is education, breathing and pressure management, movement retraining, and a home program. All of it works on camera. During the evaluation, Dr. Shalem guides a self-assessment so she can see how the pelvic floor is coordinating with breath and movement. An internal examination is not possible by video; if a hands-on assessment would change the plan, she says so and helps the patient find local care.
