How it works

How a visit works

Most of pelvic floor physical therapy is education, breathing and pressure management, and guided movement. That is why it translates so well to video.

  1. Discovery call

    15 minutes, free, by phone

    A short call to hear what is going on, explain how visits work, and check insurance or pricing. No pressure to book.

  2. Evaluation

    60 minutes, in your home or by video

    Dr. Shalem takes a full history, watches how you move and breathe, and guides a self-assessment so she can see how the pelvic floor is working.

  3. Your plan

    Same visit

    You leave the evaluation with a plain-language explanation and a short home program, usually a few minutes a day.

  4. Follow-ups

    45 minutes, every 1 to 2 weeks

    Progress checks, program updates, and techniques you can do yourself, for as long as they are useful.

Common questions in Florida

Do you take my insurance?

We are in-network with Aetna and Medicare in Florida. For other plans, you pay EMS Therapy directly and receive a superbill to submit for out-of-network reimbursement. Bring your plan name to the discovery call and we check your benefits.

How does reimbursement work with a superbill?

A superbill is an itemized receipt with the codes insurers need. You pay EMS Therapy directly, submit the superbill to your insurance company, and they reimburse you according to your out-of-network benefits.

Many PPO plans reimburse a portion after the deductible. Before the first visit, call your insurer and ask about out-of-network physical therapy benefits.

Do I need a referral in Florida?

No referral is needed to start. Florida allows physical therapy without a physician referral for the first 30 days of care. If your plan or continued care requires one, we help you get it from your physician.

Where do you offer in-home visits?

Across Miami-Dade County, including Aventura, Miami Beach, Brickell, Coral Gables, and Doral. Outside that area, video visits are available anywhere in Florida.

Can I use my HSA or FSA?

Yes. Physical therapy is a qualified medical expense, and HSA and FSA cards are accepted.

What does a visit cost?

With Aetna or Medicare, your plan's copay or cost share for physical therapy. We check your benefits before the first visit so there are no surprises.

For private pay, we share the rates on the discovery call. The rate is the same in your home or by video, and you receive a superbill after each visit.

Does pelvic floor physical therapy work over video?

Much of pelvic floor PT is education, breathing and pressure management, movement retraining, and a home program, and all of it works on camera. During the evaluation Dr. Shalem guides a self-assessment so she can see how the pelvic floor is coordinating.

An internal examination is not possible by video. If a hands-on assessment would change the plan, she will say so and help you find local care.
